File a Notification of Suspected Misconduct

If you have information on the suspected misconduct within an administrative body (ministry, government-owned company, other government entities), please complete the form below. 

This information will be processed with the utmost confidentiality, in accordance with the National Ordinance Personal Data Protection.

Fill in the questionnaire to submit a notification of suspected misconduct to the Integrity Chamber.
Anyone can file a notification of suspected misconduct. However, this cannot be done anonymously. You will be required be provide your name and other personal details in order to submit this notification.
